Report a COVID-19-Related Health and Safety Concern

This form is available to enter a report about those electing not to comply with our public health directives.  We need to model the behavior we expect from all of us. Wear face coverings, practice social distancing, engage in regular hand washing and avoid large groups.

Please supply as much information as you can. Even just a location and time can help us identify trends on our campus to improve our protocols.

Important Note: If you elect to submit a report without a name and telephone number in the “Your Information” section and/or supporting documentation in the designated section, this limits the College’s ability to respond and/or hold the student(s) accountable. If you wish to remain anonymous, you can indicate this at the bottom of the form. Even for reporters who request to remain anonymous, including at least a telephone number and supporting documentation from the reporter helps the College follow up. Our offices will make every effort to maintain anonymity, but anonymity cannot be fully guaranteed. We strive with each report to balance our commitment to public health with offering a fair process to each student.

Additionally, it is not required to have a student's name or organization to submit this form, however a lack of name(s) limits the College’s ability to respond in a timely manner and hold the student or organization accountable.
